ADMISSION POLICIES 

Policy 

  1. Students must meet the following criteria  
    • Be at least 18 years of age 
    • Read and write English 
    • Possess the inherent qualifications, skills, and knowledge required for an assisted living caregiver. 
    • Able to set a schedule for training as a priority and dedication for class completion.  Schedule for independent study as well 
  2. Make timely, if not immediate testing for the certificate. 
  3. Be aware that requirements For a Certified Caregiver job includes the following. Students are encouraged to accomplish these along with ALTA training, to be job ready upon passing the D & S Headmaster exam. 
    • Food Handler’s Certificate (must have to complete the course) 
    • CPR/FIRST AIDE 
    • TB Clearance 
    • Fingerprint Card 

 

CRITERIA FOR TRAINING PROGRAM COMPLETION  

POLICY 

  1. To complete the ALTA training students must do as following 
    • Complete 50 hours of instruction as per the NCIA approved training curriculum. 
    • Complete 12 hours of supervised skills training on the topics in the approved training program. 
    • Successfully completing quizzes for each module.   It is encouraged that a grade of 85% is achieved.  This will help ensure preparation for the D &S Headmaster exam which needs a passing grade of 75%. 
    • Reading ability is critical in order to pass a written exam. 
  2. As of 12/01/2023 the D &S Headmaster exam will be comprised of medication knowledge. However, training is required for all subjects. 
  3. The D & S Headmaster, board approved, examination is required after the ALTA training. 
    • A student is given three opportunities to test. 
    • If the student fails three times, the student must repeat the course.  It is at the discretion of ALTA as to the fees related to repeat a course. 
  4. Completion is the training, and the exam.  Then ALTA will issue a Certified Caregiver Certificate. 
  5. If there is a delay of completion within 5 months, the student records will be filed under non-completion and there will be a new start-up process including additional fees.  This is needed for the additional staff work.  
  6. Students will be notified of their non-completion status.  
  7. It is important that students test and complete in a timely manner in order to maintain retention of information while testing. 

 

STUDENT FEES  

POLICY 

  1. The student fee is $550.00 
  2. Student tuition will pay for all materials necessary to complete the course.  Directions for printing information, required for study, will be provided. 
  3. ALTA is open to discussing fees for particular circumstances i.e. multiple students, facility contracts,  multiple purchase. 
  4. When additional materials are requested, a related fee will be charged. 
  5. Students are expected to complete the course within a three-month time.  If there is a delay or a new continuance is needed the student will be charged an additional $100 and need to restart the course. 

 

GRADING, EXAMINATION, AND RE-EXAMINATION 

POLICY 

It is encouraged that training quizzes and tests achieve 80%-85% 

PROCEDURE 

  1. Quizzes will be given at the conclusion of each module. 
  2. If there is not a passing grade of 80%, the student will need to re-study the module and pass a new quiz with differing questions as the first quiz. 
  3. Students are expected to register for the D & S Headmaster exam immediately upon finalizing the class.   
  4. ALTA will support will support all actions for accomplishing a certificate, if the student are actively pursuing in the first two-moths following course completion. 
  5. The D & S Handbook has all information pertaining to the examination.  Read for a relevant information. 
  6. Testing at the board approved site will require a 75% grade to pass.  If the initial test is failed, a second and third re-examination is required. 
  7. The examination fee for caregiving is $65.00 and is required for each testing. 

 

ATTENDANCE

POLICY 

All students must complete 62 hours of curriculum and skills training. 

PROCEDURE 

  1. Students will be monitored for completion of the time requirement. 
  2. A skills checklist will be maintained for all demonstration of the ability to do the skills portion of the training. 
  3. If the course is not completed within a three-month period or in-person skills is missed as scheduled, it is at the discretion of the instructor to arrange for new continuance or a makeup session. 
  4. Students must set the training course as a priority.  It is to be a time commitment for both students and the ALTA staff. 

 

STUDENTS RECORDS 

POLICY 

A complete and accurate training record will be maintained for students in accordance with the Arizona State Law 

PROCEDURE 

  1.  Student records will be maintained in a confidential manner. 
    • Access will be restricted to instructors, state board representatives, and program administrative staff. 
    • Students may request a copy of their record by submitting a request, in writing, to the program director. 
  2. The following information will be maintained in the student record. 
    • Name, date of birth, social security number, phone, address.   As per student registration. 
    • Training record form. 
      • Attendance record, quiz record, completion score 
      • Completion of skills checklist. 
  3. Copy of food handler’s certificate 
  4. State board certified test results 
  5. Copy of the certificate issued to the student 
  6. A complete training program evaluation 
  7. Records will be kept for three years, following the student’s completion, in a fire-proof safe. 

 

PROGRAM RECORDS 

POLICY 

Training program records will be retrained in accordance with the Arizona Assisted Living Caregiver Training Program rules 

PROCEDURE 

  1. Training materials will be kept for three years. 
    • Curriculum and course schedule for each student cohort, 
    • Results of state-approved written and manual skills testing. 
    • Evaluation firms completed by students, a summary of student’s evaluations. 
    • Any measures taken to effect improvements based upon evaluations. 
  2.  Copies of all board reports, applications, or correspondence. 

 

STUDENT GRIEVANCES 

POLICY 

Students will have an opportunity to voice grievances and receive a response in a timely manner. 

PROCEDURE 

  1. Grievances may be submitted verbally or in writing.  The grievance will be documented in writing by the receiver. 
  2. The program director will be notified of any/all grievances. 
  3. The response from the director will be within 14 days, 
  4. When there are instructors, as well as the director, the chain of command will be to notify the instructor and follow-up with the program director. 

 

WITHDRAWAL AND DISMISSAL 

POLICY 

Students will be encouraged and assisted to complete the training program in accordance with the highest ethical standards. 

 PROCEDURE 

  1. Withdrawal will be assumed under any condition as previously stated.  It is imperative that students finish what is started. 
  2. Dismissal: Students will be dismissed from the program at the discretion if the director for the following. 
    • Committing acts that endanger him/herself or others. 
    • Substantiated allegations of cheating on an examination or misrepresentation of acquired skills. 
    • Committing criminal actions. 
    • Other reasons deemed worthy of dismissal by the director. 
    • Not-attending or completion of testing.  Parameters are discussed previously. 
    • Prior to dismissal, students will receive the following: 
      • First offense- documented verbal warning. 
      • Second offense – written warning. 
      • Third offense – Dismissal from the program.
