Admission Policies
Policy
- 1. Students must meet the following criteria.
- a. Be at least 18 years of age
- b. Read and write English
- c. Possess the inherent qualifications, skills, and knowledge required for an assisted living caregiver.
- d. Able to set schedule for training as a priority and dedication for class completion. Schedule must allow time for independent study as well.
- e. Completion will also include acquiring a food handler’s certificate before testing.
- 2. Make timely if not immediate testing for certificate.
- 3. Be aware that requirements for certified caregiver job includes the following.
- Fingerprint clearance card before enrollment
- Students are encouraged to accomplish along with ALTA training, to be job ready upon passing the D&S headmaster exam.
- a. CPR/FIRST AIDE
- b. TB Clearance, adhering to current CDC guidelines
- c. Food Handler’s Certificate
Criteria for Training Program Completion
Policy
- 1. Complete the ALTA training as follows, students must
- a. Complete 50 hours of instruction on the topics in the approved training program curriculum. (written curriculum in person quizzes, online instruction/quizzes are available)
- b. Complete 12 hours of supervised skills training on the topics in the approved training program curriculum.
- c. Independent reading and vocabulary study will be assigned additionally. Participation will be required in these assignments.
- d. Successfully completing quizzes for each module. A grade of 75% is required. However, extra study is recommended if 85% is not acquired for any module.
- e. Reading ability is critical, and emphasis will be put on reading comprehension.
- 2. Headmaster test will be comprised of medication knowledge. However, training is still required for all required subjects. Basic caregiving is emphasized with current vocabulary study
- 3. A Board-approved examination, given by a board approved provider will be required after ALTA training.
- a. A student must pass and is given two more opportunities for retesting.
- b. If the student fails three times, the student must repeat the course. It is at the discretion of ALTA as to the fees related to a repeat course.
Attendance
Policy
All students must complete 62 hours of study in accordance with the established training guidelines. Every effort is made by ALTA to accommodate an agreed upon schedule.
Procedure
- 1. All students will be monitored for class progress record as stated for attendance. This record will be reviewed for ascertaining completion of required class time.
- 2. The training record will be reviewed daily by the instructor, and initialed for completion and evaluation.
- 3. A skills checklist will be maintained for all demonstrations of ability to do skills portion of the class.
- 4. If a student misses scheduled classroom or skills assessment, it is at the discretion of the instructor to arrange for makeup training.
- 5. Students must set attendance at the training course as a priority. It is to be a time commitment for both students and staff.
Grading and Reexamination
Policy
Students must test at 75% for board test. Students must pass quizzes with a 75% grade.
Procedure
- 1. Quizzes will be graded at the end of each quiz. Evaluation will be done at the end of grading.
- 2. If the quiz is not passed with a 75% grade, then an additional quiz, with the same competencies examined in the original will be given. The additional quiz will have items different from those in the original quiz.
- 3. Students will be advised as to completion of the subject or encouraged for further study.
- 4. Testing at the board approved site will require a 75% grade to pass. If the initial test is failed, a second and third re-examination is allowed.
- a. Students will be expected to register for testing immediately upon finalizing the class. If additional study time is required, the student is advised to accomplish this with the instructor.
- b. All quizzes and test results are part of the student record.
- 5. Reexamination.
- a. Reexamination addresses the same competencies examined in the original examination.
- b. Contains questions that may differ from those on the original examination.
- c. Is documented in the student’s record.
- d. ALTA will support all actions needed for accomplishing a certificate, if students are actively pursuing in the first two months following completion of the course.
- 6. If students require additional support, paper documents, assistance after the first two months, there may be a fee for later support.
Student Records
Policy
A complete and accurate training record will be maintained for students in accordance with Arizona state law.
Procedure
- 1. Student records will be maintained in a confidential manner.
- a. Access will be restricted to instructors, state board representatives, and program administrative staff.
- b. Students may request a copy of their record by submitting a request in writing to the program director.
- 2. The following information will be maintained in the student record.
- a. Name, date of birth, social security number, phone, address.
- b. Training record form.
- 1. Attendance record including any needed make-up.
- 2. Test and quiz scores. Notation of needed additional study or retests.
- c. Completed skills checklist.
- 3. Copy of food handler’s card.
- 4. State board certified test results
- 5. Copy of the certificate of completion issued to the student.
- 6. A completed training program evaluation.
- 7. Records will be kept for three years, following the student’s completion, in a secure fire-proof safe.
Program Records
Policy
Training program records will be retained in accordance with Arizona Assisted Living Caregiver Training Program rules.
Procedure
- 1. Training materials will be kept for three years.
- a. Curriculum and course schedule for each student cohort.
- b. Results of state-approved written and manual skills testing.
- c. Evaluation forms completed by students, a summary of student’s evaluations.
- d. Any measures taken to effect improvement based upon the evaluations.
- 2. Copy of all board reports, applications, or correspondence.
Student Fees and Financial Aid
Policy
- 1. Student fees are nonrefundable.
- 2. Financial aid is not available currently. However, after a $100.00 registration fee, a payment program can be established to pay off the tuition fee.
- 3. Employment contracts or other agreements can be made to effect training program payment.
- 4. The student fee is $550 for individual training. Discounts can be given for two or more.
- 5. Student tuition will pay for the following.
- a. All classroom materials necessary to participate in skills training.
- b. Student handbooks.
- c. Student manuals and binders.
- 6. When additional reports and information are requested, a materials fee will be charged accordingly.
- 7. If a student discontinues, and wants to resume training at another time, an additional fee, no less than $100 and dependent on material needing to be reviewed, will be expected to be paid.
Withdrawal and Dismissal
Policy
Students will be encouraged and assisted to complete the training program in accordance with the highest ethical standards.
Procedure
- 1. Withdrawal: Students who voluntarily withdraw from the program have 2 months to re-enroll and complete the remainder of the training. An additional fee will be charged as stated under payments.
- 2. Dismissal: Students will be dismissed from the program at the discretion of the director for the following reasons/directions.
- a. Committing acts that endanger him/herself or others.
- b. Substantiated allegations of cheating on an examination or misrepresentation of acquired skills.
- c. Committing criminal actions.
- d. Other reasons deemed worthy of dismissal by the director.
- e. Not-attending or completion of testing.
- f. Prior to dismissal, students will receive the following:
- i. First offense – documented verbal warning
- ii. Second offense – Written warning signed by the student
- iii. Third offense – Dismissal from the program
Student Grievances
Policy
Students will have an opportunity to voice grievances and receive a response in a timely manner.
Procedure
- 1. Grievances may be submitted verbally and in writing.
- 2. The Manager will be notified of any/all grievances.
- 3. The response from the Manager will be within 14 days.
- 4. When there are instructors, as well as managers, the chain of command will be to notify the instructor and follow-up with the program director/Manager.
