ALTA came about because there was a need for a certified manager. Maybe , I will train one. That was the start of developing a training program with the hope of providing the ever increasing need of quality caregivers and managers for Assisted Living Facilities.

SCOPE OF SERVICES
Our facility provides the following services that include but not limited to: Supervisory, Personal, Directed, Dementia, Alzheimer’s, Therapeutic Diet, and Hospice. We will offer Behavior Care but not Behavior Services (as the primary diagnosis of care) Respite and adult daycare.
THE FOLLOWING ARE THE STANDARD SERVICES PROVIDED BY THIS FACILITY
RN owned facility
Staff 24 hour certified caregiver, additional according to need.
Staff will sleep overnight
Nighttime monitoring, as required and alerting system
Or an overnight awake staff
Three meals and snacks daily
Room with all facility cleaning and maintenance
Medication administration (authorized as needed for level of care)
Or other medication organization and as needed for suggested vaccines.
Laundry and supplying of all linens, towels.
Standard showers are twice weekly or as needed
Complete assistance with activities of daily living (ADL’s)
Incontinence care every two hours or as needed
Room/bathroom furnishing bed bureau storage, shower chair
Scheduled Group Activities and Individual Activities
Coordination with HCP, health care providers and home health services,
coordinated by a Managing RN (PT, OT, Social Work)
ALTC services and required communication.
Helping with outside services and arranging transportation through providers
Scope of services can be subject to change dependent upon specific needs of a resident and falling within the scope of regulations for this licensed facility.
SCOPE OF SERVICES
This facility is to function as a Residential Health Facility as regulated by A.R.S. Article 7. Offering Behavioral Health Services. Day Care for non-overnight. Respite Care. Residents accepted 18 years of age or older
The following is offered as well as each resident receiving an individual treatment plan.
RN owned and supervised, and administrated
On-going assessments
Group therapies, specific to treatment plan; relapse prevention, mindfulness, coping skills, Cognitive
Behavioral Therapy, GMH, SA, SMI
Personal counseling
Continuous protective oversight
10 bed capacity
24-hour staff by Behavioral Health paraprofessional, Behavior health Technician, or certified caregiver
depending on resident’s needs
All homemaking functions are performed for the resident with encouragement to participate.
Three meals and snacks. Provided or supervised resident preparation
All linens for beds and showering/bathing
Laundry
Recidivism Reduction plans and coordination with established programs
Medication administration and/or supervision of self-administration
Co-ordination with accredited outdoor programs
Recreational programs, equipment and outings
In addition to the scope of services above, each client has a treatment, following an assessment and as per the facility Behavioral Health Professional
